Subacute Care

 

An intermediate level of care provided to individuals who require more intensive medical supervision and rehabilitation than what is typically offered in traditional post-acute care settings but less than acute hospital care.

 

Transitional Care:

Services focused on supporting patients as they move between different levels of care, including subacute care, to ensure a smooth and effective transition during recovery.

Rehabilitation Therapy:

Therapeutic interventions, such as physical therapy, occupational therapy, and speech therapy, provided in subacute care settings to promote recovery and improve functional abilities.

 

Long-Term Acute Care Hospital (LTACH):

Specialized facilities that provide extended medical and rehabilitative care for patients with complex medical conditions who require a longer length of stay than traditional hospitals.

 

Post-Acute Care:

Comprehensive healthcare services and support provided to individuals after an acute illness or injury, often including subacute care, rehabilitation, and transitional care.

 

Respiratory Therapy:

Specialized care focused on the assessment and treatment of respiratory conditions, commonly provided in subacute care settings for patients with respiratory challenges.

Wound Care Management:

Specialized attention to the treatment and healing of wounds, including surgical wounds or pressure ulcers, within subacute care facilities.

Patient and Family Education:

Informative sessions and resources provided to patients and their families in subacute care settings to enhance understanding of medical conditions, treatment plans, and recovery expectations.

 

Palliative Care:

Supportive care focused on improving the quality of life for patients with serious illnesses, often integrated into subacute care to address physical, emotional, and spiritual needs.

Intermediate Care Facility (ICF):

Residential facilities that provide subacute care and support services for individuals with chronic illnesses or disabilities who do not require hospitalization but need more assistance than a typical nursing home provides.
