Hospitalist

A physician who specializes in the care of hospitalized patients, coordinating treatment plans, monitoring progress, and communicating with other healthcare providers involved in the patient’s care.

Nursing care plan

A customized plan developed by nurses for each patient, outlining the specific care interventions and goals tailored to the individual’s condition and needs during their inpatient stay.

Discharge planning

The process of preparing a patient for leaving the hospital, which includes coordinating post-discharge care, medication instructions, follow-up appointments, and ensuring a smooth transition to home or another healthcare setting.

A digital version of a patient’s paper chart, containing their medical history, diagnoses, medications, treatment plans, immunization dates, allergies, radiology images, and laboratory test results, accessible to authorized healthcare providers.

Hospital-acquired infection (HAI)

Infections acquired by patients during their stay in a healthcare facility, often due to exposure to pathogens in the hospital environment or through invasive procedures.

Patient Satisfaction Score

A measure of how satisfied patients are with their overall inpatient care experience, typically assessed through surveys or feedback forms, which helps healthcare facilities identify areas for improvement.

Transitional Care

Services provided to patients as they transition from one healthcare setting to another, such as from hospital to home or from hospital to a rehabilitation facility, to ensure continuity of care and prevent readmissions.
